Job Summary Position: Community Healthcare Advocate Job Summary: The Community Healthcare Advocate works directly with individuals and communities to facilitate access to healthcare services, health education, and resources. They serve as a liaison between healthcare providers, community organizations, and individuals, particularly those facing barriers to healthcare access. The advocate empowers individuals to make informed decisions about their health, ensuring they receive necessary care, prevention services, and follow-up support. Responsibilities Educate members about the importance of regular health screening, health checkup, and preventive care. Assist members in navigating the health system, including but not limited to coordination of care. Connect members to community resources, such as housing assistance, healthcare, and food insecurity programs. Provide education to members and caregivers to address systemic barriers that may prevent members from accessing needed support. Identify and engage hard-to-reach populations to engage them in health services. Provide immediate assistance to navigate during emergency health situations. Timely monitor and follow up on member’s progress and support plans as necessary by conducting virtual/in-person timely outreach. Maintain detailed and confidential records as per HIPAA compliance. Work with the team to provide coordinated and comprehensive care. Work with the administrative staff to provide support in the day-to-day operational activities. Engage with local communities to raise awareness about available healthcare services. Conduct informational sessions on health promotion, disease prevention, and healthy living. Provide culturally sensitive health education to diverse populations. Advocate for patients with healthcare providers to ensure appropriate care. Qualifications A degree in public health, social work, nursing, or a related field is required. At least 2-3 years of experience in healthcare, community outreach, or social services. Experience working with diverse populations and knowledge of health disparities and community-based resources. Cultural competence and the ability to work with people from diverse backgrounds. Knowledge of healthcare systems, insurance, and public health programs. Excellent communication and interpersonal skills to include the ability to develop relationships with members and caregivers.
